The name on your certification is automatically generated from the profile information in your account. If you set up your account under the wrong name or under a nickname, this will create a certificate in the incorrect name. Likewise, if you input the incorrect state for your exam location, this will be incorrect on your certificate.

To request a certificate correction, please email help@alwaysfoodsafe.com with the following information:

  • The email address related to your Always Food Safe account
  • The information on the certificate and correction
  • The reason for correction
